In order to provide a professional, motivating and enjoyable experience for everyone, please read through the following Terms and Conditions for Personal Training with Jessica DiBiase. If you have any questions please do not hesitate to ask.
Payment. For all training sessions, including but not limited to private training, group classes, and body work, payment must be paid in full before client(s) can begin.
Scheduling & Cancellations. Please provide at least 24 hours notice if you need to cancel or reschedule your appointment(s). Email confirmation is required for cancellation. No refunds or rescheduled cancellations can be made with less than 24 hours from the time of the appointment. Please note that Buddy Sessions are charged in full in the event that only one person can make the appointment.
Group Training/Classes. If a member of the group cancels or misses an appointment that group member will forfeit that session regardless of any prior notice given. No credits or refunds will be given for missed group classes unless otherwise stated.
Trainer Cancellation of Missing an Appointment. In the event of unexpected absence, your trainer will contact you as soon as possible. If your trainer is absent due to a case of an emergency you will be contacted to explain the situation and reschedule the session with no penalty to the trainer.
Weekend Classes at Albany Gym Location.. Please book your class sessions with Jessica online via Sweat Health’s website.
Missed Training Sessions Due to Injury. Jessica will issue credit for any missed sessions due to injuries. Injuries must be documented by a physician’s note in order to receive credit. Credit must be used within 14 days after receiving medical clearance to exercise.
Returning to Exercise after Injury. You must provide documentation for medical clearance to return to exercise. No exceptions.
Vacations and Absences: Please provide 7 days advance notice of any prolonged training absences.
Trainer Vacations and Absences. Jess will provide you with 14 days notice for any prolonged trainer absences.
Health Concerns. You must inform your trainer of any relevant injury or illness and will obtain a medical clearance from your medical specialist prior to beginning (or restarting) training if there are any medical concerns.
Medical Freeze Option. If you become ill or injured and have been advised by your doctor to temporarily discontinue training, you may freeze any remaining sessions for a period up to 3 months.
Liability. You accept that participating in exercise has a risk of causing injury, both minor and potentially major.You accept this risk and accept full responsibility for your exercise and training. You will in no way hold Jessica DiBiase liable for any injuries or illness sustained. You also agree that you have no undisclosed injury or illness that may affect your ability to undertake rigorous exercise and to the best of your knowledge you are ready and able to undertake this exercise program.
Please review and sign the Waiver Of Liability Form and present the signed document to Jessica prior to your first training session.
